Empower families and strive to ensure the health, success and safety of those we serve

“Success is to be measured not so much by the position that one has reached in life as by the obstacles which he has overcome while trying to succeed." - Booker T. Washington

 ABOUT US

The Chairman, President and Chief Executive Officer of 7TH & Madison Extensions of Empowerment ensures all employees share a common drive and passion when delivering services. She was born and raised in New Jersey and has over a decade of experience in the Horizon Blue Cross Blue Shield system.

The founder began her health insurance career with Horizon Blue Cross Blue Shield of New Jersey before starting her own agency with the co-founder, hence the creation of 7TH & Madison Extensions of Empowerment in New Jersey. At Horizon BCBS, she served as a Suspense Quality Analyst while investigating and resolving executive level inquiries. Both founders were able to apply passion to their vision, hence creating the first Case Management Organization (CMO) for adults with Developmental and Intellectual Disablities located in Newark, New Jersey. Subsequently they moved their offices to Maplewood, New Jersey.

In addition, the founders work with a number of charitable and community organizations in the state of New Jersey. She is also President of an organization that works directly with the American Cancer Society which provides discounted human hair wigs to individuals that are low income and/or socio-economically disadvantaged. Our founder also serves as a mentor to individuals that are developmentally/intellectually disabled by providing internships and employment opportunities as well.

At 7TH & Madison Extensions of Empowerment LLC. we pride ourselves in taking a more traditional, compassionate and hands on approach to our families while providing case management services, prevocational training, community based supports, translation services, community inclusion, and behavioral management for people with intellectual and developmental disabilities who are eligible for DDD services. 7TH & Madison staff are responsible for utilizing a person-centered planning process to identify outcomes, planning goals, and support needs and develop/maintain the Individualized Service Plan (ISP). In addition, 7TH & Madison staff also help the person and his/her family find supports and services that best meet his/her needs and preferences. The Support Coordinator, in particular also conducts ongoing monitoring of the provision of services included in the New Jersey Individualized Service Plan.

7TH & Madison also provides Intensive In-Community services (IIC) and Behavioral Assistance (BA) services, which provide flexible, multi-purpose, in-home/community clinical support services for parents/caregivers/guardians and children/youth with behavioral and emotional needs who are receiving case management or Out-of-Home services. The purpose of these interventions is to strengthen the family, to provide family stability and to preserve the family constellation in the community setting. Behavioral Assistance Services are specific, outcome-oriented interventions designed to augment other mental/behavioral health services included in a child/youth’s comprehensive plan of care. 7th & Madison Behavioral Assistance Service is a dynamic process of intervention designed to target specific behaviors that are interfering with a child/youth’s functioning. It includes ongoing evaluation resulting in effective modification of a child/youth’s identified behavior(s). They are components of an approved, written, detailed individualized behavioral assistance plan of care prepared by a licensed clinical behavioral health care practitioner.
