7TH & Madison ​Specialist provide encouragement, and assistance for members, with or without the caregiver present, in or out of the member's residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined by his/her Individual Service Plan. 

Community-Based Services are delivered one-on-one with a member and may include but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, money management, computer lab access, transportation training/awareness, ordering from a menu, going out to restaurants, increasing social skills, and activities of daily living. 

7TH & Madison Achievement Program offers a unique approach, we develop custom programs specific to the areas our member's and their families wish to increase socialization skills, communication skills, independence, and self-esteem.
